Registering With Lindfield FC Juniors

By signing our Club registration form you confirm that you have the read and agree to the details below regarding medical conditions, personal accident cover and membership fees:

Medical Conditions
Lindfield F.C. Juniors, and those acting on the club's behalf (from herein 'The Club'), will take the necessary first aid action required should an affiliated member of The Club become injured in the normal course of a game or training session. However, The Club cannot take responsibility for administering any specialist medical treatment or device should a member require such medical attention. In these cases, a parent or guardian should always be available to administer such treatment and take the necessary action required and not hold The Club responsible for not administering or mis-administering specialist treatment.

It is the responsibility of all parents or guardians to inform The Club of any special medical requirements that a member may have on the club's medical form. In these circumstances, The Club will discuss with parents or guardians, in the strictest confidence, medical action required and will endeavour to work with them to ensure the safety of their children and all other members within the club.

In the interest of safety of players at training or matches, the club needs to be aware of any medical conditions that your child may have. Such information may be important to anyone called upon to treat an injury. Please inform your team manager of any details that you think may be relevant.

Youth Personal Accident Cover
Lindfield FC Juniors have taken out personal accident insurance for all players aged 5 - 16 inclusive, as recommended by the Sussex FA. Should you require additional insurance cover for your child we would recommend that you source separate cover either through their school or on an individual basis. The level of cover is as follows:

Accidental death: £5,000
Permanent Total Disablement: £12,500
Loss of one eye and/or limb: £5,000
Loss of both eyes and/or two limbs: £15,000
Dental cover - flat benefit: £100
Fracture benefit (8 weeks) per week: £15

This applies to all games, travel and organised training but excludes injury resulting from alcohol and/or substance abuse and any pre-existing conditions.

Membership Fees

Lindfield FC Juniors has recently registered under the Inland Revenue's Community Amateur Sports Club Scheme (CASC). One of the benefits of this registration to the Club is that individuals who are UK tax payers can make gifts to CASCs using Gift Aid in the same way as they can make gifts to charities. This will provide a significant increase to the level of income for the Club and therefore this income will be available for the future benefit of all members.

However, such Gift Aid is only available for gifts to CASCs and not for other payments such as membership subscriptions. Therefore, it has been agreed by the Committee that the amounts payable by members will comprise two elements - a subscription to cover registration and affiliation costs of the Club and a donation to the Club, for which Gift Aid can then be claimed. Both amounts are payable - the donation element is NOT optional.

Amounts payable for the 2006/07 season are as follows:

TeamSubscription Element(25%)Donation ElementTotal Payable
Non-league registered players£16£49£65
League registered players£25£75£100

It should also be noted that if the donor pays tax at the Higher rate they can claim additional relief in their Self Assessment return.
